Our Programs

John Muir Health supports vibrant and healthy communities through strong partnerships with non-profit organizations, schools, clinics, and other community-based providers to address our community's most pressing needs. Our priority areas are identified through a Community Health Needs Assessment every three years, which is informed by input from community stakeholders.

Selection Criteria

John Muir Health focuses its efforts on programs that address vulnerable populations, which include those with:

  • Evidenced-based disparities in health outcomes
    • Such as Black maternal health inequities, increased rates of diabetes in the Latino populations, or youth mental health outcomes
  • Significant barriers to care
    • Including isolation, frailty, language or cultural barriers, transportation or mobility challenges, and difficulty voicing healthcare needs
  • Economic disadvantages
    • As experienced by uninsured, underinsured, and working poor residents

John Muir Health partners with non-profit organizations throughout Alameda and Contra Costa Counties to serve our community's most pressing needs.
